Surgical correction of lens changes in Marfan’s syndrome

Abstract

Introduction. Ocular symptoms of Marfan’s syndrome are usually associated with lens impairment. There are three typical eye manifestations of this disease: weakened zonules, myopization and lens ectopy. At the beginning of the process visible signs are absent and there is no necessity for a surgical approach; so only optical correction of the induced ammetropy is usually prescribed. Lens ectopy progression is the only indication for a surgery.

Purpose. To describe one of the possible lens surgical correction in Marfan’s syndrome cases in order to perform intraocular optic lens (IOL) implantation – phaco-aspiration with capsular bag reposition and intracapsular IOL implantation.

Material. The surgical operation was performed on 4 eyes of 2 patients aged 7 and 26 suffering Marfan’s syndrome.

Results. The surgery required several step-by-step manipulations: tunnel incision of the cornea in its limbal zone, two corneal paracentesis, anterior continuous curvilinear capsulorhexis 4,5– 5,0 mm in diameter, hydrodissection of crystalline lens mass, aspiration of crystalline lens mass, intracapsular ring implantation, capsular bag reposition by means of sutural ring fixation to the sclera, flexible IOL implantation with capsular bag fixation.

Conclusion. Proposed in this study surgical approach is microinvasive and permits intracapsular IOL fixation in patients suffering Marfan’s syndrome-associated lens impairment.
