The difficulties of diagnosis and treatment of gangrene Fournier for one more time

Abstract

Fournier’s gangrene (necrotizing fasciitis of perineum, perianal area, scrotum and penis) is widely described in Russian and foreign literature. Despite the fact that the unified approach to diagnostics and treatment of this life-threatening disease is developed, mortality is still up to 75%, and reaches 100% if a patient had not undergone a surgery.

We report in this article a clinical case of an atypical distribution of the necrotizing process that effused to the left retroperitoneal space, paranephric fat and a diaphragm that we had never met in our practice, and available literature.

Timely diagnostics, radical surgery tactics, multidisciplinary approach to the treatment based on current knowledge about etiology and pathogenesis of this disease in the one hand, and our own experience in the other hand, had not been able to prevent a fatal outcome.
