Neuromodulation in the treatment of chronic non-malignant pain (from the clinical guidelines on “Surgical treatment of chronic neuropathic pain syndrome”)

Abstract

Less effectiveness of conventional treatment for chronic pain syndromes is one of the most frequent indications for neurosurgical methods. In medical literature and medical legislation of Russia there are no official clinical guidelines, regulating the use of neurosurgical treatment for non-malignant chronic pain. Particularly, neurostimulation methods (peripheral nerve stimulation, deep brain stimulation, spinal cord stimulation, motor cortex stimulation) are approved only for typical neuropathic pain syndromes. At the same time such methods like occipital nerve stimulation, subthalamic stimulation, sacral nerve root stimulation, are not included in the official recommendations on the treatment for such syndromes like chronic essential headache (chronic cluster headache, chronic migraine) and chronic pelvic pain. This publication is the project of Russian official recommendations for the use of surgical neuromodulation in the treatment of chronic non-malignant pain syndromes. Common methods and types of surgical neuromodulation are considered. Indications, contraindications and patient selection criteria are described for each neuromodulation method.
