Remodeling effects of universal chordal-sparing mitral valve replacement
Abstract
The necessity of preserving annulopapillary interaction during mitral valve replacement is an accepted fact. However, as far as the technique and extent of chordal-sparing mitral valve replacement are concerned there has been no such unanimity. The authors have developed a novel universal chordal-sparing mitral valve replacement, and based on their clinical experience of performing this surgical technique in 508 patients, they analyze its remodeling effects on left ventricular geometry and function: decrease in volume of the inflow portion and preserved separation of inflow and outflow tracts of the left ventricle; optimization of isovolumic form of contraction by a uniform movement of the graft to the left ventricular apex before the onset of ejection; elimination of a risk of right ventricular regional contractile impairment.
