Direct results of treatment of gunshot neck wounds

Abstract

Aim. Specialized help to patients with neck wounds is among the most challenging issues of modern surgery. However, in the conditions of the local military conflicts with gunshot wounds treatment in city hospitals has some specific features.

Material and methods. The analysis of 129 cases of gunshot neck wounds is the basis for research. The age of wounded subjects varied from 18 to 65 years. All patients were on treatment in a surgical hospital of city hospital # 9, Grozny, the Chechen republic. All subjects got their wounds during local military operations in the Republic of Chechnya during 1991–2000 years.

Results. Analysis of the immediate postoperative period in patients with gunshot neck wounds demonstrated that it proceeded hard, complications occured in 51,9% of cases during follow-up. The main complications registered in patients of this group were purulent – septic events which developed in 21,7% of cases. Large number of purulent – septic complications may be explained by several factors: firstly, patients came to medical institution without qualified medical care at a pre-hospital stage. Secondly, there was absence of necessary diagnostic tools in medical institutions working in extreme conditions of military operations, and it often led to diagnostic mistakes. Thirdly, severity of wounds should also be considered. The lethality was 32,5%. Hemorrhagic shock (13,1%) was the main reason of lethality in patients with gunshot neck wounds.
