Influence of hybrid approach on success rate of chronic total occlusions recanalization

Abstract

Aim. Chronic total occlusions (CTO) of the coronary arteries are detected in more than a third of patients with significant lesions. Often, the presence of occlusion is one of the main reasons for performing coronary artery bypass grafting (CABGs). The development of the technique of interventional recanalization over 20 years, the usage of a modern instrument and the hybrid approach are conditions for achieving a high percentage of technical success near 90%.

Material and methods. During the period from 2014 to 2017 in Federal Center for Cardiovascular Surgery (Astrakhan) 261 endovascular interventions on CTO of coronary arteries were performed. Average age of patients was 58±8.6 years, mainly men 216 (83%). Patients were divided into groups before introduction of hybrid approach (2014–2015) – 123 interventions and after introduction of hybrid approach (2016–2017) – 138 interventions.

Results. Recanalization of coronary arteries СTO success rate in the pooled group was 211(81%). As a result of hybrid approach success was achieved in 117 (85%) cases, in comparison with the previous period when successful were 94 (76%) cases (p=0,049).

Conclusion. The usage of a hybrid approach in our practice allows to achieve success rate of recanalization up to 85% even in patients with complex anatomical options of coronary arteries chronic total occlusions.
