Pulmonary thrombendarterectomy with two temperature regimens in patients with chronic thromboembolic pulmonary hypertension

Abstract

Background. Today, pulmonary thromboendarterectomy surgery shows good early and long-term results worldwide. However, the search for the optimal perfusion continues, ensuring the bloodless surgical field and maximum protection of organs and systems.

Material and methods. The article compares 2 groups of patients operated under conditions of deep (18–20 °C) and moderate (21–24 °C) hypothermia using intermittent circulatory arrest. The 1st group included 35 patients, the 2nd group – 15 patients. Groups of patients were similar. All patients underwent isolated pulmonary thromboendarterectomy.

Results. The hemodynamic results after surgery were excellent in 2 groups of patients. Among the complications, no significant differences were observed.

Conclusions. The operation in conditions of moderate hypothermia can be recommended as an alternative to the standard method. But the care should be taken.
