Prognostic value of metalproteinases for the immediate postoperative period in the cases of closed spleen injuries

Abstract

Aim – to define value of metalproteinases as a prognostic factor for the immediate postoperative period in the cases of closed spleen injuries.

Material and methods. 96 patients with the closed spleen injuries underwent operation. The isolated damages were revealed in 47 (49%), combined – in 22 (22.9%) cases. According to it all patients were divided into two groups: 47 patients with the isolated injuries of a spleen were included in the 1st group, 22 patients with the combined trauma were included in the 2nd group. We used AIS (Abbreviated Injury Scale) to assess the weight of damages. The following indicators applied: fabric inhibitor metaloproteinaz-1, matrix metalloproteinase-2, -7, -9 in blood serum. Blood sampling was carried out from a peripheral vein on days 1, 3, 5, 7, 10 and postoperatively. 

Conclusion. Metalproteinases play an important role in development of complications in patients with closed spleen injuries. Changes of TIMP-1 and MMP-2, -7, -9 in serum of blood differ and depend on severity of the patient. So, in isolated injuries of a spleen, the most expressed changes were noted in group of patients with III degree damage according to AIS , the least changes – in patients with I degree damage. At the same time, restoration of the studied indicators happened quicker in group of patients with I degree damage who recovered for 5 postoperative days whereas patients with III degree damage recovered for 13 postoperative days. Other results were registered in group of patients with combined and multiple damages: in this group changes in the analyzed indicators were noted from the postoperative day 1, and complete recovery did not happen even on postoperative day 13.
