Step-by-step treatment of secondary valvular cardiomyopathy followed by simultaneous intervention for colorectal cancer and paresis of the right dome of the diaphragm

Abstract

Cardiovascular and oncological comorbidity is a frequent observation in age-related patients, which requires special diagnostic alertness. When identifying such a life-threatening combination of two pathologies, there is a problem of priority-the stage of surgical intervention, the justification of the simultaneous approach and the assessment of the degree of risk.
Here is the medical history of a 72-year-old patient who underwent two heart surgeries under cardiopulmonary bypass for severe cardiopulmonary failure against the background of progressive valvular dysplasia and secondary cardiomyopathy within one year. In the course of these therapeutic measures, cancer of the transverse colon was detected and a simultaneous operation was performed: resection of the transverse colon with lymph node dissection in combination with plastic surgery of the right dome of the diaphragm, paresis of which developed after the first cardiac surgery.
