The results of the early postoperative period after correction of total anomalous pulmonary venous connection

Abstract

Aim - to compare obstruction of the pulmonary veins and mortality in the early postoperative period with various methods for correcting total anomalous pulmonary venous connection. 

Material and methods. In the presented pilot, two-center, simple, blind, prospective randomized study, the quality of life of patients after correction of total anomalous pulmonary venous connection in infants was evaluated using two methods: the sutureless repair (n=20) and conventional repair (n=20). The total sample size was 40 patients.

Results. Hospital mortality was only in the group of conventional repair in 4 (20%) patients (p=0.106). The predictors of death were the pulmonary vein score, pulmonary artery pressure in the early postoperative period, and any gradient on the anastomosis. Severe obstruction on the anastomosis was also only in the group of conventional repair in 6 (30%) patients (p=0.02). The only risk factor for obstruction of the pulmonary veins was newborns <2.5 kg.

Conclusion. Obstruction of the pulmonary veins depends on the variants of correcting total anomalous pulmonary venous connection. The sutureless technique reduces the incidence of residual obstruction from 30 to 0%. The only predictor of obstruction of the pulmonary veins was the weight <2.5 kg in newborns.

The sutureless technique reduced the frequency of mortality after correction of total anomalous pulmonary venous connection from 20 to 0%. Independent predictors were the pulmonary vein score, pulmonary artery pressure in the early postoperative period, and any gradient on the anastomosis.
