Transverse artery of the neck as an alternative source of revascularization of tissue complexes in reconstructive surgery of the face and neck (clinical experimental study)

Abstract

One of the most difficult issues in microsurgical autotransplantation of maxillofacial tissues is the choice of recipient vessels for safe revascularization of flaps in cases of severe scar tissue deformations from previous operations, extensive lymphodissection with ligation of the external carotid artery, and radiation fibrosis, which makes secondary reconstructions of maxillofacial tissues practically impossible.

The purpose of our work: to study the topographical and anatomical features of the external triangle of the neck with the development of access to the transverse artery of the neck.

A total of 30 unfixed corpses aged 62 to 85 years were examined with the study of topographic and anatomical features of the transverse artery of the neck from the pool of the thyroid trunk.

During the period from 2017 to 2019, a total of 6 autotransplantation of tissue complexes with alternative revascularization through the transverse artery was performed.

The transverse artery of the neck and the thyroid trunk basin is a reliable and safe source for arterial revascularization, located below and laterally to the external carotid artery and less susceptible to atherosclerotic, post-radiation and scar changes.
