Surgical repair of intermediate atrioventricular septal defect, double orifice of left atrioventricular valve in adult patient with dextrocardia: a case report

Abstract

We describe a case of surgical repair of intermediate type atrioventricular septal defect, common atrium, double orifice of left atrioventricular valve in 29-year-old female with situs inversus and history of recurrent sinusitis.

Left atrioventricular valve was replaced by mechanical prosthesis, atrium septum was restored by two patches, coronary sinus was abandoned on the left side. The patient was extubated at same day and discharged at day 11 after surgery. One year later any rhythm disorders were not revealed, mechanical prosthesis function was good, SaO2 - 95%, woman had NYHA class I-II.

The combination of an intermediate type atrioventricular canal defect, common atrium, a double orifice of the left atrioventricular valve, an anomaly of the confluence of systemic veins, and a situs inversus is not common variant anatomy of the congenital heart defect. The initial total correction undertaken at the age of 29 years makes this case to be a rare clinical observation.
