Use of polypropylene explants in gastrostomia in experiment

Abstract

Enzyme systems, cellular features, the functioning of the digestive tube obey the conveyor principle, therefore, interference in the work of the digestive system during gastrostomy, in addition to surgical complications, is accompanied by the consequences of violation of this principle, which determines the relevance of the study.

Aim – to develop a gastrostomy method that reduces the risk of complications by using a mesh.

Material and methods. Prospective experimental study in 18 rabbits. The main group (n=6) – gastrostomy by the author’s method. 1st comparison group (n=6) – Witzel gastrostomy and 2nd comparison group (n=6) – Depage–Janeway gastrostomy using the GIA apparatus. For histological studies and morphometry, 5 animals were bred from each group on days 10 and 20.

Results. Microcirculation disorders and necrosis in the area of sutures (with gastrostomy according to Witzel and Depage–Janeway) are leveled by using a mesh. More pronounced morphological changes in the form of microenvironment necrosis and stagnant microcirculation disorders occurred after operations without using a mesh.

Conclusion. The use of a mesh causes tissue rearrangements due to fibrillogenesis, which contributes to more reliable fixation of the gastrostomy, and a decrease in the mechanical load on the ligatures.
