The use of a large omentum in the surgical treatment of hernias of the esophageal orifice of the diaphragm and Barrett’s esophagus
Abstract
Background. Hernia of the esophageal opening of the diaphragm and
Barrett’s esophagus represent a special group of pathologies in surgery. The
choice and improvement of methods for surgical correction of these diseases is
a particular problem.
Aim. To apply a new method of surgical treatment of hiatal
hernia and Barrett’s esophagus.
Material and methods. During the development of the method, experimental
operations were carried out in 4 cats and 2 dogs. Also, the method was tested
using cadaver material. The studies were carried out in accordance with the
ethical requirements for working with experimental animals and approved by the
ethical committee.
Results. Comparative characteristics with already known
methods of treatment were carried out: fundoplication according to Nissen,
Tupe, Dor, Chernousov with selective proximal vagotomy. The essence of the
obtained method lies in the use of a greater omentum to form the true angle of
His.
Conclusion. The proposed method of treatment with the use of
flaps of the greater omentum allows to preserve the anatomical structure as
much as possible, reduce the incidence of complications and accelerate the
recovery time.
