Combined myoplasty and omentoplasty of the anterior chest wall

Abstract

Aim. To demonstrate possibility of combined plastic surgery of the anterior chest wall with various tissues.

Material and methods. The treatment of two patients with poststernotomy mediastinitis was analyzed. After debridement of the wound, in both cases, a reconstructive operation was performed, including the use of the greater omentum and pectoralis major muscles in the first case, and the combination of the pectoralis major muscle and the rectus abdominis muscle in the second case.

Results. In both cases, the postoperative period was uneventful. Wound healing was the first intention. Patients were discharged with convalescence. A good long-term result was noted after 6 and 12 months.

Conclusion. In both clinical cases chronic sternal infection (fistulous tract with exudation) presented. The limitation of the process and the low level of microbial flora made it possible to perform debridement of the wound simultaneously with the reconstructive operation. The optimal material for the reconstruction of the chest wall with poststernotomy mediastinitis is the greater omentum and muscle flaps (pectoralis major and rectus abdominis).
