Peristomal skin disorders in patients with digestive ostomies

Abstract

Аim: the study was to optimize diagnostics and preven- tion of peristomal skin disorders in ostomy patients.

Methods. On the 1st stage frequency and character of peristomal skin disorders were analyzed before and after introduction of the developed procedures of diagnosing and prevention of in 956 and 409 complications of the digestive ostomies in patients of corresponding groups. On the 2nd stage causes of peristomal skin disorders and their impact on the quality of life was investigated. The algorithm for diagnosis of peristomal skin disorders and the DET score, the Ostomy Skin Tool, digital photos, measuring grid for assessing the area of skin changes and the Stoma QOL tool.

Main results. The frequency of peristomal skin disorders was: 20,5 and 11,7% in first and second groups respectively. Contact dermatitis prevailed in both groups: in 19,3 and 9,8% respectively observations. Main causes of peristomal skin disorders are: stoma complications in 41,9% patients, improper care of the stoma in 25,6% pa- tients, peculiarities of bowel function in 20,9% patients, skin diseases in 9,3% patients, allergy in 2,3% patient. It was found that Life Quality Index at patients with peristomal skin disorders is much lower, than in group of patients without complications: 41,7±8,8 and 57,8±9,1 respectively (р<0,05).

Conclusion. Peristomal skin disorders negatively affect the quality of life of ostomy patients. Introduction of a complex procedure of diagnosing, prevention and treat- ment helped to reduce frequency of peristomal skin disorders from 20,5 to 11,7% (p<0,05).
