Laparoscopic cholecystectomy from two accesses – experience of domestic authors

Abstract

Actuality. In last years in Russia 2-ported surgery for cholelithiasis deservedly and rightly is gaining in popularity. This is accomplished by operations according to methods SILS+ and by using of standard laparoscopic instruments. The article presents the experience of performing of different versions of 2-ported laparoscopic cholecystectomy in our country. A new method of 2-ported laparoscopic cholecystectomy is described. The purpose of the study is to improve the results of surgical treatment of patients with cholelithiasis and gallbladder's polyposis by developing and implementing a new method of 2-ported laparoscopic cholecystectomy.

Material and methods. According to this method were operated 127 patients. 118 (92,9%) patients had cho- lelithiasis and 9 (7,1%) – polyposis of the gallbladder. Chronic cholecystitis was detected in 78 (66,1%) cases and acute – in 40 (33,9%) one.

Results. 2-ported laparoscopic cholecystectomy was successfully performed in all cases. There were no cases of conversion. The proposed method can be applied in patients with BMI over 30 kg/m2 and destructive forms of cholecystitis.

Conclusions. 2-ported laparoscopic cholecystectomy according to the original method is the perspective and safe low-invasive method of surgical treatment of cholelithiasis. This method is comparable in convenience of execution with standard technique and excels its at cosmetic results. The developed method is economically perspective and rational due to use of standard laparoscopic instruments. It is easily reproducible.
