Cholecystectomy through a single laparoscopic approach – a new «gold standard» for the treatment of the gallbladder diseases?

Abstract

Currently, laparoscopic cholecystectomy is the most common operation in the world. A number of studies were designed to clarify the effect of reducing the number of incisions on the anterior abdominal wall at the early and late postoperative results. Against this background, the invention of a single laparoscopic approach (SILS) as a method of performing laparoscopic surgery, has become very actual. Unfortunately, at the moment in Russia there are only a few papers analyzing the results of cholecystectomy through a single laparoscopic approach, most of which describes one operation type «SILS + 1» – with the introduction of an additional trocar. That is why we present to you the results of performing 117 laparoscopic cholecystectomies through a single laparoscopic approach.

The aim of the research was to evaluate the results of performing laparoscopic cholecystectomy using SILS.

Results. All 117 operations were performed successfully. There was no conversion to conventional laparoscopic or open cholecystectomy. Operative time was 39 min (29–78). SILS techniques in the treatment of diseases of the gallbladder allows almost completely refuse to use non-narcotic analgesics. It is noted reduction in the duration of hospital stay in the postoperative period up to 1 day. All 117 operations were performed successfully, there were no intra- and postoperative complications.

Conclusion. SILS cholecystectomy is a safe operation, easily reproducible in the hands of an experienced surgeon.
