Results of usage of laparoscopy in abdominal trauma patients

Abstract

Relevance. Nowadays the level of diagnostic mistakes, postoperative complications and mortality remains high in abdominal trauma patients. At the same time management of such patients is a source of controversy.

The aim of our study was to determine indications and contraindications for usage of laparoscopy in abdominal trauma patients, to decrease the amount of explorative laparotomies and to improve technique of laparoscopic revision of abdominal cavity.

Materials and methods. A total of 348 patients with abdominal trauma were included in the survey in the period of 2010–2014; 186 (53,4%) of them had blunt trauma and 162 (46,6%) patients had stub wounds to abdomen. US (ultrasound) examination was used as a screening test in all these patients.

Results. Explorative laparotomies were avoided in 56,6% of the patients and laparoscopic operations were performed in 21% of the patients. There were no missed injuries in our study.

Conclusion. The results of US examination were used to define indications for emergency laparoscopy. The usage of laparoscopy in abdominal trauma patients allowed avoiding explorative laparotomies in 77,6% of the cases. The study was retrograde, level of evidence was II.
